question?

Hi.am going to cdc tonight and just wondered what the normal procedure is,do i have to get any forms signed by my doctor.

You only need a form signed by the doctor if your BMI is over 40 or if you are on any medication

If you choose to do the SS option a letter is sent to your doctor to inform him of your decision, this is only so your medical notes can contain all relevant info about you
