Ongoing Check-Up Form

Dearest knowledgeable peoples!

I'm going to try and get my check up done today by a local pharmacy. I've been emailed the form from lighterlife and am not sure what I should fill in before I take it to the pharmacy.

The top section with name, address, dob, CIF number (no clue what this is) - is this for me or for who ever is doing my test?

All help appreciated!!!:confused:
 
Well i went along anyway to get my blood pressure done, anyway I'm in the ideal category which is always pleasing.

Anyway the pharmacist refused to sign the LL form as she couldn't guarantee that I wasn't on any medication!! Anyway I've got the top copy of their form which shows that Lloyds pharmacy did it and my readings. She's also signed and dated that.

Do you think this will be sufficient for my LLC?
 
Thats something i would like to know too....

Not had to have a check yet as ive only completed one week!

Our LLC never has any problem taking forms as long as they confirm blood pressure and heart rate. If you're concerned, give your LLC a call or e-mail and he/she should answer this for you easily.

By the way, your CIF number is your client number and it doesn't matter if you don't put it on the form. If you need to know it, it should be noted on your record book inside the front cover, or if not you'll have received some correspondence from LL with your client number on it.

Yeah should be fine. They usually attach it to their form. Funny how some pharmacies are so anal about those things and others are absolutely fine.
I have a nurse in my group so she does the blood pressure for us there and then :)

I used to get mine taken at a pharmacy or the doctor, whichever I could fit into my schedule best. There are now some Superdrugs who are endorsing LL and are licenced by them to sign off start forms and do the checks - I know there are a few in SE London now as I saw the posters in my LLC's office.
