Refeeding sachets and pharmacists

When I read all of the LT literature, it said that when you begin refeeding you have some shakes and some food (all carefully introduced) and that you continue going to the pharmacy to be weighed. The pharmacist then helps find your balance, but initially having 14 shakes a week, then down to 7, and after a couple of weeks on 7 shakes a week (ie 1 a day) this can be altered. Therefore if you're losing weight still on 7 shakes, they reduce you to 5 shakes, and if you begin to gain, you get 9 shakes for the following week and so on.

Couple of questions -
A/ Do you have to go every week, or can you try say 7 shakes a week for a fortnight / month before going back to be weighed?

B/ Are these shakes the same as the TFR ones, or do you get moved onto maintenance shakes? (In which case, do people really bother going in, or do they monitor themselves at home?)

C/ How much do they cost? I figure they must have a 'per shake' figure and work it out individually, but I thought I caught a glimpse at my pharmacy of a pricing sheet that had a men's weekly price, women's weekly price, and maintenance at "£12 per unit" - how much would a 'unit' be? 7 shakes I assume (as 21 are £36)?

D/ How do the maintenance shakes differ from TFR, and would it be possible to do your own slow refeed / maintenance using those if it was difficult to get to the pharmacy? What about price?

Long way off refeeding but I want to know what lies ahead!
