If you want to do SS (that's 3 packs a day), you need a letter. However if you want to do SS+ or higher, you don't. Welcome to the dark side ;)

ETA - Alternatively if your LLC still has your letter, you may (and I don't know if it'll work) be able to get a copy of her.

Is there a big difference between SS and SS+? How many shakes/bars etc so you have on SS+ as opposed to SS?

since your bmi is under 40 you wont need a form signed by your gp unless you have a medical condition that is contraindicated.

your cdc will have to write to your gp to inform them if you are on ss or ss+
 
On SS you get 3 meals (shakes, tetras, soup, porridge, bar)
On SS+ you get either 4 meals + 200ml of skimmed milk, or 3 meals and a 200 cal me'real food' meal (protein + 2 tbsp low carb veg)
